Movement confidence check

Count one point for each “Yes.”

1. Do you feel less steady when walking or changing direction?
2. Is getting up from a chair harder than it used to be?
3. Have you reduced activities because of strength, balance, or confidence?
4. Do stairs or uneven surfaces make you cautious?
5. Does stiffness or limited mobility interfere with daily activities?
6. Do you tire more quickly during walking, errands, or recreation?
7. Would objective testing help you feel more confident about what to work on?

0–1: Few reported concerns; testing can still establish a useful baseline.

2–3: An assessment may reveal early opportunities to improve.

4 or more: The Elevate 65 Assessment could be especially valuable.
